- Copy of Birth Certificate
- National Identification Number (NIDA)
- Copies of Secondary Education Certificates (O-Level and A-Level)
- Valid License for Upgrading Students (if applicable)
- Recent Passport-Size Photo – 360x300mm, with a light blue background
- Indexing Number issued by the Tanzania Nursing and Midwifery Council
